Anatamopathologie

A l'histo, on observe :

  • Atrophie corticale asymétrique prédominant en frontopariétal et rolandique
  • Perte neuronale et gliose sévère corticale et sous-corticale
  • Neurones ballonisés et achromatiques
  • Accumulation de protéine tau avec plaques gliales et dégénérescence neurofibrillaire

Clinique

Non spécifique, apparition lente de :

  • Syndrome parkinsonien initialement asymétrique (++ "maladresse" d'un MS / "main étrangère" et troubles de la marche avec discrète instabilité posturale), répondant peu/ pas à la L-dopa
  • Tremblement postural et d'action souvent intriqué à des myoclonies
  • Fréquents phénomènes dystoniques des membres atteints, assez évocateurs lorsqu'associés à un Sd parkinsonien
  • Apraxie (++ idéo-motrice)
  • Troubles cognitifs modérés et syndrome frontal généralement à l'arrière plan mais parfois inauguraux
  • Parfois : troubles sensitifs, dysarthrie assez tardive, syndrome pyramidal, mvts choréoathétosiques, dysphasie, dysphagie, troubles oculomoteurs (ophtalmoplégie supranucléaire fréquente mais discrète par rapport à la PSP), troubles psy et comportementaux

En pratique, le diag est à évoquer devant l'association d'un syndrome parkinsonien asymétrique non dopa-sensible à une dystonie d'un membre/ une apraxie asymétrique/ des myoclonies focales/ des troubles de l'équilibre tardifs

Examens complémentaires

  • IRM cérébrale : aN tardives et peu Sp. Le plus évocateur : atrophie fronto-pariétale asymétrique
  • PET : hypométabolisme asymétrique prédominant en fronto-pariétal et thalamique

Les examens complémentaires, trop peu Sp, ne présentent aucun intérêt pour le diag positif. Leur utilisation (biologie, IRM, testing neuropsy, PL,...) ne se justifie que dans le cadre d'un diagnostic différentiel, variable selon la présentation clinique (cf syndromes démentiels - généralités, syndromes parkinsoniens, tremblements, mouvements anormaux,...), pour exclure des causes curables.

Prise en charge

  • Médicale : PEC des myoclonies et dystonies (++ clonazépam), efficacité réduite de la l-dopa chez certains patients pour le Sd parkinsonien
  • Paramédicale : cf Alzheimer
