Démences associées à d'autres maladies neurodégénératives
- Dégénérescence cortico-basale
- Paralysie Supranucléaire Progressive
- Atrophie multisystématisée
- Chorée de Huntington
- Démence présente à 100% + chorées-athétoses + troubles psychiatriques. ++ 30-50 ans.
- Maladie de Parkinson
- Les troubles cognitifs sont fréquents d'ans l'évolution, généralement à l'arrière plan des troubles moteurs. Toujours exclure une cause curable intercurrente (++ iatrogène) de dégradation. Une véritable démence, ++ tardive et de mauvais pronostic, seraient observée dans 30-40% des cas.
- Sclérose Latérale Amyotrophique SLA
- Démence dans 15%, suivant ou précédant la SLA, ++ de type fronto-temporal
- Aphasie primaire progressive fluente :
- atrophie temporale gauche, histologie semblable à Alzheimer, apparition rapide d'autres troubles cognitifs, évolution vers une démence de type Alzheimer
- Amnésies dégénératives isolées
- Exceptionnelle. Longue préservation des autres fonctions cognitives. Atrophie hippocampique bilat avec hypométabolisme temporal interne
- Atrophie corticale postérieure (syndrome de Benson)
- Atrophie bilat, ++ prédominante à D, temporo-pariéto-occipitale. Développement progressif de : agnosie visuelle, désorientation topographique, simultagnosie, alexie, agraphie spatiale, syndrome de Gerstmann, ataxie optique, syndrome de Balint, déficit campimétrique / extinction visuelle. Le langage est relativement préservé, la mémoire est normale, pas d'anasognosie. Débute généralement par des troubles visuels peu Sp.
- Divers : troubles cognitifs divers (apraxies gestuelles, agnosie visuelle, syndrome de Gerstmann, syndrome du gyrus angulaire dominant, prosopagnosie, aprosodie progressive, amnésies pures,…) avec démence d'apparition tardive en relation avec une atrophie focale
Démences infectieuses irréversibles
HIV (encéphalopathie tardive à HIV, leucoencéphalopathie multifocale progressive), maladies à Prions, séquelles d'encéphalite herpétique ou d'autres (méningo-)encéphalites,…
Etiologies diverses
RXth pan-encéphalique (encéphalopathies radiques), la majorité des démences vasculaires, Chth, trauma crâniens graves, post neurochir, maladie de Fahr, Sd de stress post-traumatique (! rares véritables démences décrites, sur toxicité des hormones de stress/ coticoïdes endogènes ? – traiter systématiquement les troubles dépressifs), post greffe hépatique/ rénale ou shunts porto-cave, post-anoxiques, sur hypo/hyperglycémies prolongées/ chroniques, SEP (troubles cognitifs dans l'évolution de ≈ 60% des patients),…
